Blood Flow Restriction Training (BFRT) & EMS
Blood flow restriction training and electrical muscle stimulation are low-load strengthening tools used at IP Physio by chartered physiotherapist Ilias Paschopoulos at our Soho, Marylebone, Canary Wharf and Notting Hill Gate clinics in London, for patients who cannot yet tolerate the heavy loading normal strength gains require.
Preventing the strength loss that follows surgery
Muscle atrophy begins within days of surgery or serious injury. The traditional response, wait until the tissue tolerates normal loading and then rebuild, leaves patients emerging from the protected phase with substantial deficits that take months to reverse.
Blood flow restriction training uses a calibrated cuff to partially restrict arterial inflow and limit venous return while you exercise at 20 to 30% of your maximum. The resulting metabolic environment triggers strength and hypertrophy responses comparable to heavy resistance training, at loads a healing tissue or a fresh surgical repair can accept.
Pressures are individualised to your limb occlusion pressure rather than applied as a fixed setting, and sessions are supervised. Used correctly it is a well-tolerated, well-evidenced way to protect muscle during the exact period when you would otherwise lose it.
Where EMS fits
Electrical muscle stimulation drives contraction directly through the muscle rather than relying on your own neural drive. That matters most in the first weeks after knee surgery, when quadriceps inhibition can make voluntary activation genuinely difficult regardless of effort.
We use EMS for early post-operative quadriceps activation, for neuromuscular re-education where a muscle is switched off after nerve irritation or prolonged immobilisation, and combined with voluntary contraction to improve the quality of activation rather than as a passive treatment on its own.
Both tools are bridges. They protect and rebuild capacity until you can load properly, at which point progressive resistance training takes over.
Our objective-data approach
Cuff pressure is set from your measured limb occlusion pressure, so the stimulus is individualised and repeatable rather than a generic setting applied to every patient.
Dynamometry tracks whether the strategy is doing its job: we retest the affected muscle group regularly and expect to see maintained or improving force while heavy loading is still off the table.
ForceDecks testing is used once loading resumes, to confirm that the strength preserved with BFRT and EMS is translating into real power and load acceptance rather than isolated gym numbers.
Prices and insurance
£90 to £175 per session. Sessions range from £90 to £175 depending on clinic and appointment length, and a block of 10 sessions carries a 10% discount.
