    TMJ Physiotherapy

TMJ physiotherapy at IP Physio treats temporomandibular joint dysfunction, jaw pain, clicking, locking and associated headaches. It is delivered by chartered physiotherapist Ilias Paschopoulos at our Soho, Marylebone, Canary Wharf and Notting Hill Gate clinics in London, assessing the jaw, neck and contributing habits as one system.

    Why jaw problems get missed

Jaw pain, clicking, stiffness or tension affects eating, speaking, sleep and concentration, and many people arrive having already seen several practitioners, been told the joint looks structurally normal, and still have daily symptoms nobody has explained.

That happens because the jaw is usually examined in isolation. The temporomandibular joint does not work alone: it is mechanically and neurologically linked to the upper cervical spine, the masticatory and suboccipital muscles, breathing pattern, sleep position, bruxism and stress load. Assess the jaw without those and the picture is incomplete.

Our assessment covers jaw opening range and deviation, joint sounds and their timing, disc behaviour, muscle tenderness and trigger points in masseter, temporalis and pterygoids, upper cervical mobility, and the daytime and night-time habits that keep loading the joint.

    What treatment involves

Treatment typically combines intra-oral and extra-oral soft tissue work, joint mobilisation for the TMJ and upper cervical spine, targeted control and range-of-motion exercises for the jaw, and cervical strength and mobility work where the neck is contributing.

Alongside the hands-on work we address the behavioural drivers: clenching awareness and habit reversal, resting tongue and jaw position, chewing load, sleep posture, breathing, and how stress cycles into muscle tension. These are what stop symptoms returning once the joint settles.

Where a dental or maxillofacial opinion is needed, for a splint, a suspected structural problem or unexplained changes in bite, we say so and coordinate rather than persist with physiotherapy alone.

    Our objective-data approach

Jaw range of motion is measured in millimetres, opening, lateral excursion and protrusion, so change is tracked numerically rather than by impression, and deviation patterns are recorded at every reassessment.

Cervical strength and endurance testing, including handheld dynamometry for the deep neck flexors and extensors, quantifies the neck contribution that so often drives jaw and headache symptoms.

Structured symptom and function tracking, pain on chewing, headache frequency, locking episodes, gives us a reliable curve to judge whether the plan is working within the expected timeframe.
